What is longevity?

The term "longevity" describes not only lifespan (Lifespan), i.e. how many years a person lives, but mainly theHealthspan: the number of years a person lives in good health, without chronic diseases and with full function. The goal of modern aging science is not "to add years to life", but "to add life to years", i.e. to compress the period of illness to the end of life as much as possible.

This distinction is central: two 70-year-olds can be in completely different biological states. The difference between them is measured throughbiological age, and not through the age on their ID card.

The Hallmarks of Aging

In 2013, researchers (López-Otín et al., in the journal Cell) proposed a unifying scientific framework for the aging process, updated in 2023 to 12 "hallmarks of aging". These are the cellular and molecular processes that accumulate with age: DNA damage, telomere shortening, epigenetic changes, declining mitochondrial function, cellular senescence, low-grade chronic inflammation, and more. The practical conclusion: aging is not a single decree of fate, but a series of processes, some of which can be slowed.

The seminal paper (from 2013, updated 2023) defines the central biological processes of aging. It forms the scientific basis for the modern Longevity field and the search for interventions that slow these processes.

What the science shows really works

Contrary to marketing promises, there are a few lifestyle factors for which the scientific evidence is strong and consistent:

Cardiorespiratory fitness (VO2max)

Of all the measures, aerobic fitness is one of the strongest predictors of lifespan. Large studies found a consistent link between high fitness and significantly lower mortality.

Muscle mass and strength

Muscle mass and grip strength are linked to function and independence in older age and to lower mortality. Resistance training is one of the important components, especially from age 40 and up.

Metabolic health

Insulin sensitivity, normal glucose, and normal lipids (ApoB) are a foundation for longevity. Simple blood tests reveal risk years before disease appears.

A large study found a strong link between aerobic fitness level and lower mortality risk - the higher the fitness, the lower the risk, without a clear "ceiling" observed. This finding positions aerobic fitness as one of the strongest levers for longevity.

What is less established, and important to know

A large part of the Longevity market is based on supplements and promises for which the human evidence is still weak or partial. The rule is simple: lifestyle interventions (movement, sleep, nutrition, metabolic health) are the proven foundation. Supplements are at best a marginal addition, not a substitute. A critical, evidence-based approach is worth more than any "next trend".

How to start

The first step is measurement, because you cannot improve what you do not measure. It is best to start by understanding your current state through biological age and biomarkers, and only then build a personal, staged protocol.
